Welcome and thank you for choosing Blue Shield of California

On this site you can find your health plan documents, which you can download below, to distribute to your employees.

You are required to provide a uniform Summary of Benefits and Coverage (SBC) and Evidence of Coverage/Certificate of Insurance (EOC/COI) to participants and beneficiaries for Medical and Dental plans that you offer. You must also distribute SBCs to covered dependents when the dependent’s last known address differs from the employee’s.

What you need to do

You may ensure electronic distribution of these documents to your Blue Shield subscribers by:
(1) posting in a read-only format on an intranet site which is accessed by your employees, or
(2) emailing directly to your Blue Shield subscribers.

If you choose to post your health plan documents on an intranet site, your employees must be notified that their health plan documents are available to review, download, and print from your intranet site. You must also provide reasonable and appropriate instructions to access and print the documents from your intranet site.

Please download your group's Blue Shield health plan documents below for distribution to your eligible Blue Shield members. Be sure to follow the full instructions above on how you must distribute them. We will automatically update these documents if and when new ones become available.
